Why tenderness may occur

Breast tissue can respond to changing oestrogen and progesterone during perimenopause, particularly when periods are becoming less predictable. Tenderness may feel aching, swollen, heavy, prickly, or sore when touched, and it can affect one or both breasts. A bra that no longer supports comfortably, chest-wall strain, or a benign cyst can add to the discomfort.

Breast tenderness is not a reliable way to identify menopause by itself. Hot flushes, night sweats, mood changes, vaginal dryness, low sex drive, or periods stopping may occur during the transition, but other conditions and medicines can cause similar symptoms. Any persistent focal change needs its own assessment.

Ways to manage discomfort

Note when tenderness appears, whether it follows a cycle, and whether a new medicine or activity preceded it. Wear a supportive, well-fitting bra, use a warm or cool compress, and consider simple pain relief only if it is suitable for you. Ask a pharmacist or clinician about interactions and medical conditions before taking anti-inflammatory medicines. Keep routine breast checks and screening appointments according to local guidance.

When to seek care

Book a prompt examination for a new lump, skin dimpling, nipple inversion, bloody or clear spontaneous discharge, persistent one-sided pain, or a change that does not settle. Seek urgent care for a red, hot, rapidly swollen breast with fever or feeling very unwell. Mention hormone therapy, contraception, pregnancy possibility, and family history during the assessment.
