What may be happening
During teething, drooling and changes in feeding can coincide with softer or more frequent stools. Wetness, rubbing, and contact with stool can then irritate the skin beneath the nappy. The rash may look red, sore, or bumpy on the cheeks of the bottom and around the nappy area.
Teething does not explain every rash. Contact Dermatitis may follow a new wipe, nappy, detergent, or cream. Atopic Dermatitis can affect other dry, itchy areas, while a sharply spreading rash, blisters, crusting, or spots outside the nappy area may need a clinician to consider infection or another cause.
What to do at home
Change wet or soiled nappies promptly. Rinse with lukewarm water or use fragrance-free wipes, pat dry rather than rubbing, and apply a thick layer of bland barrier ointment. Give short, supervised nappy-free periods when practical, and use loose clothing to reduce heat and friction.
Avoid fragranced products, talc, scrubbing, and experimenting with several medicated creams. If the rash is not settling, a pharmacist or doctor can recommend a suitable treatment after seeing it. Wash hands before and after changes, especially when the skin is broken.
When to seek care
Contact a clinician if the rash is worsening, painful, cracked, bleeding, blistered, or not improving with careful nappy care. Seek urgent help if your baby has fever, seems unusually sleepy, is feeding poorly, has breathing trouble, or develops rapidly spreading redness or facial swelling. A rash after a new medicine also needs prompt advice.
