Can syphilis spread through saliva?

Syphilis spreads through direct contact with an infectious sore or rash, including lesions inside the mouth that may be painless and hard to notice. During oral sex, contact between mouth tissue and an infectious genital or anal lesion can transmit infection. Kissing may carry a risk when an infectious sore is on the lips or inside the mouth; saliva without contact with a lesion is not considered the usual mechanism. Ordinary sharing of drinks, food, or cutlery does not spread syphilis.

Arrange STI Screening if you had oral sex or close mouth contact with a partner who may have syphilis, particularly if either person had a mouth sore, genital sore, or rash. Describe the contact and timing so the clinician can choose appropriate tests and decide whether repeat testing is needed. Avoid sexual contact while waiting for advice. If infection is confirmed, complete the prescribed Antibiotic Treatment and follow guidance about partners and follow-up.

Seek prompt review for a mouth sore, genital sore, rash, swollen glands, or known exposure. Obtain urgent care for eye pain, vision changes, hearing changes, severe headache, confusion, weakness, or difficulty walking. Pregnancy or possible exposure during pregnancy should be discussed with a clinician without delay.

Ask whether the type of mouth contact needs testing, when a result will be reliable, whether a mouth or genital examination is needed, when intimacy can resume, and whether partners need assessment.
