How can syphilis affect the face?

A syphilis sore can occur on the lips, inside the mouth, or around the face after contact with an infectious lesion. It may be painless and can heal without treatment. A later rash may involve the face or other parts of the body, sometimes alongside swollen glands, fever, or feeling unwell. Many other conditions can cause facial spots, sores, or irritation, so a photograph or appearance cannot confirm the cause.

Rarely, syphilis can involve the eyes, ears, or nervous system. Eye pain, blurred or reduced vision, hearing change, severe headache, weakness, or confusion are more urgent than an ordinary rash.

Arrange STI screening for an unexplained mouth or facial sore, a new widespread rash, or sexual contact with someone who has syphilis. Avoid picking or sharing items that touch the lesion, and avoid sexual or oral contact until assessed.

Tell the clinician when the change began, whether it is painful, and whether you have had genital or anal symptoms, fever, swollen glands, or a diagnosed partner. If testing confirms syphilis, take the prescribed antibiotic and follow advice about partners and follow-up.

Seek urgent care for eye pain, sudden or worsening vision changes, facial weakness, severe headache, confusion, or new hearing loss. Seek prompt assessment for a mouth sore that does not heal or a rash after a possible sexual exposure.

Ask whether the lesion needs direct examination, which STI tests are appropriate, whether your eyes or ears need checking, and how to protect partners while results and treatment are arranged.
