What should you know about syphilis by kissing?

Syphilis spreads through direct contact with an infectious sore or lesion. A sore in the mouth, on the lips, or in the genital or anal area may be painless and easy to miss. Kissing may therefore carry a possibility of transmission when an infectious mouth lesion is involved; ordinary social contact such as sharing space does not spread syphilis.

A person cannot determine infectiousness from appearance alone. A visible sore may have another cause, and syphilis may cause no obvious symptoms. Risk also depends on the type and timing of contact, so a clinician should interpret the exposure rather than relying on the presence or absence of a lesion.

Arrange confidential STI screening and explain when the kissing occurred, whether there was a mouth sore, and whether other sexual contact took place. Testing may be recommended immediately or again later depending on timing. Until you have advice, avoid kissing or sexual contact with a partner who may encounter a sore or lesion.

If testing confirms syphilis, complete the prescribed antibiotic treatment and tell the clinician about recent partners. Do not assume that no visible sore means no infection, and do not use someone else’s medicine. Follow-up confirms that the care plan is complete.

Seek prompt assessment after kissing a partner with a known or suspected infectious mouth sore, especially if you develop a painless mouth lesion, rash, swollen glands, or fever. Seek urgent care for vision changes, severe headache, confusion, weakness, or trouble swallowing or breathing. Pregnancy with a possible exposure needs timely medical advice.

Ask whether the exposure warrants testing now or repeat testing, whether other STI tests are appropriate, how long to avoid close or sexual contact, whether a partner needs assessment, and which symptoms should prompt urgent review.
