What should you know about syphilis brain damage?

When syphilis involves the nervous system, symptoms may include severe or persistent headache, confusion, changes in memory or behaviour, weakness, numbness, balance problems, or difficulty coordinating movement. Eye involvement can cause pain or vision changes, while ear involvement can cause hearing loss, ringing, or dizziness. These findings can have other causes, so assessment may include a neurological examination, blood tests, and sometimes testing of fluid around the brain and spinal cord.

Neurological involvement can occur at different stages and may be present even when skin or genital symptoms are absent. A person with syphilis and neurological, eye, or ear symptoms may need a treatment plan different from treatment for uncomplicated infection.

Contact a clinician promptly if syphilis is diagnosed or possible and you notice neurological, eye, or hearing symptoms. Share when symptoms began, any recent sores or rash, prior syphilis treatment, and medicines or allergies. Follow the prescribed antibiotic plan carefully and attend every review. Do not self-treat with leftover antibiotics, because the required medicine and route depend on the affected body system.

Until a clinician gives you specific advice, avoid sexual contact that could transmit infection. Partners may need confidential testing and treatment assessment. A follow-up plan can include repeat blood tests and review of symptoms.

Go to urgent care for sudden vision loss, new weakness on one side, severe confusion, fainting, a seizure, a sudden severe headache, or rapidly worsening balance. Seek same-day assessment for new hearing loss, persistent eye pain, marked dizziness, or neurological symptoms with a positive syphilis test. Pregnancy with suspected or confirmed syphilis also warrants prompt clinical review.

Ask whether your symptoms suggest nervous-system, eye, or ear involvement; which examinations or tests are needed; whether treatment must be given in a clinic or hospital; how partners will be supported; and which changes should trigger emergency care.
