What should you know about syncope cough?

A hard bout of coughing increases pressure inside the chest and can briefly reduce blood returning to the heart. Blood flow to the brain may fall for a short time, causing light-headedness or loss of consciousness. The cough itself may have many causes, including an infection, asthma, reflux, smoking-related irritation, or another airway problem.

Anaemia, atrial fibrillation, dehydration, and hypoglycaemia can add to dizziness or collapse, but a cough-associated episode should not be self-diagnosed from the symptom alone. A clinician can separate cough syncope from a seizure, a heart rhythm problem, or another cause of blackout.

At the first sign of dimming vision or weakness, sit or lie down away from stairs, traffic, and hard edges. If another person is present, ask them to stay with you. Avoid driving, climbing, swimming alone, or operating machinery until the cause and recurrence risk have been reviewed.

Keep a record of the cough pattern, triggers, medicines, wheeze, fever, mucus, palpitations, and what witnesses saw. Do not suppress a persistent cough with borrowed medicine; treatment depends on its cause.

Seek emergency help for fainting with breathing difficulty, blue lips, chest pain, coughing blood, severe injury, confusion, prolonged unresponsiveness, or a fast or irregular heartbeat. Same-day medical advice is sensible when episodes recur, the cough is worsening, or you faint without a clear coughing trigger.

Ask whether you need an examination of your heart and lungs, an electrocardiogram, blood tests, or review of current medicines. Mention whether the blackout occurs at the peak of coughing, after standing, or without warning.
