Symptoms of spinal gout

Gout develops when urate crystals collect in and around joints. In spinal gout, deposits may affect the small joints of the spine, the spaces around the spinal cord, or areas where nerves leave the spine. Pain may be localised to the back or neck, and some people notice tenderness or pain that travels into an arm, chest, buttock, or leg. Tingling, numbness, reduced strength, or difficulty walking can occur if a nerve is irritated or compressed. A history of gout, kidney disease, or repeated attacks in other joints can provide useful context, but back symptoms alone cannot identify the cause.

Arrange a clinical assessment if back pain is persistent, unusually intense, or accompanied by a new sensory or movement change. A clinician may review your gout history and medicines, examine your strength and reflexes, and consider blood tests or imaging. Do not change urate-lowering treatment or take anti-inflammatory medicine without checking that it is suitable for your kidneys, stomach, and other medicines. Note when symptoms began, which movements worsen them, and whether swelling or pain is also present in other joints.

Seek urgent medical care for new or worsening leg weakness, loss of balance, numbness around the groin, or loss of bladder or bowel control. Fever with severe back pain, confusion, or feeling acutely unwell also needs prompt assessment because infection and other spinal emergencies can resemble a gout flare. Sudden inability to walk should be treated as an emergency.

Ask whether your symptoms could involve spinal nerves, which tests would distinguish spinal gout from infection or a disc problem, and whether your current gout treatment needs review.
