Symptoms of miscarriage with an IUD

Miscarriage may involve vaginal bleeding, cramping, pelvic pain, passing tissue, or a reduction in pregnancy symptoms, although some people have few symptoms at first. An IUD does not cause every episode of bleeding or pain, but a pregnancy that occurs with an IUD has a higher concern for ectopic pregnancy, which develops outside the womb. One-sided pain, shoulder-tip pain, dizziness, or fainting can signal internal bleeding and should be treated as an emergency.

If you could be pregnant and have an IUD, contact a maternity or urgent-care service promptly for advice, even if bleeding is light. Do not pull out the device yourself. Note when symptoms began, how the bleeding is changing, where pain is located, and whether you have passed clots or tissue. Assessment may include pregnancy testing, examination, blood tests, and a transvaginal ultrasound to locate the pregnancy and check what is happening.

Go to emergency care for severe or worsening abdominal pain, one-sided pain, fainting, marked dizziness, shoulder-tip pain, heavy bleeding, fever, or feeling suddenly very weak. These signs can indicate ectopic pregnancy, significant blood loss, or infection. Even without these warning signs, any confirmed pregnancy with an IUD requires prompt clinical review rather than waiting for symptoms to resolve.

Ask whether the pregnancy is inside the womb, whether the IUD is visible and how it should be managed, and what symptoms require immediate return. If miscarriage is confirmed, ask about expectant, medical, and surgical options and the follow-up plan.
