Symptoms of Heart Failure After Bypass Surgery

After bypass surgery, worsening breathlessness, swelling in the legs or abdomen, unusual tiredness, a persistent cough, or rapid weight gain can signal heart failure and should be assessed by a healthcare professional. Recovery itself can cause tiredness and reduced activity, so the pattern, severity and change from your usual recovery matter. Heart failure means the heart is not pumping enough blood for the body's needs; fluid may then collect in the lungs or tissues.

Breathlessness may appear during activity, when lying flat, or during the night. A cough can be dry or may occur with a feeling of chest congestion. Fluid retention can cause swollen ankles, tight shoes, abdominal fullness or a quick change in body weight. Fatigue, reduced exercise tolerance and needing more rest can occur because muscles receive less oxygen-rich blood. Heart failure can also occur alongside rhythm problems such as atrial fibrillation or with high blood pressure, but these symptoms have other possible causes after an operation, including anaemia, infection, lung problems or medication effects. Only an examination and appropriate tests can identify the cause.

Contact your cardiac or surgical team promptly if breathlessness is getting worse, swelling is new or increasing, you are waking short of breath, or your activity becomes harder than it was during recovery. Keep a record of when symptoms started, what brings them on, whether they improve with rest, and any change in swelling or weight. Take prescribed medicines as directed and do not start, stop or double a dose without clinical advice. Keep follow-up appointments so your heart rhythm, blood pressure, lungs, wound and fluid balance can be reviewed. A chronic disease review may help organise ongoing monitoring and treatment.

Seek emergency help immediately for severe or rapidly worsening breathlessness, breathlessness at rest, fainting, new confusion, blue or grey lips, or chest pressure or pain that does not settle. Sudden palpitations with dizziness, marked weakness or shortness of breath also need urgent assessment. Do not drive yourself if you feel faint or severely unwell. Call local emergency services and follow the operator's instructions. A new symptom after bypass surgery should not be dismissed as normal recovery when it is severe, sudden or progressing.

Ask what changes in breathing, swelling or exercise tolerance should prompt a call; whether your medicines, salt and fluid advice need adjustment; and which checks are planned for heart rhythm and pumping function. Tell the clinician if you have cough, fatigue, palpitations or swollen ankles, and explain whether each symptom is improving, stable or worsening. You can also ask when cardiac rehabilitation or a follow-up review is appropriate for your stage of recovery.
