Swelling Over the Knee Cap: What It Can Mean

A soft swelling directly in front of the kneecap can occur when the prepatellar bursa becomes irritated, sometimes after repeated kneeling. A knee injury may affect the ACL or meniscus and lead to fluid inside the joint, often with pain, stiffness, catching, or instability. Knee Osteoarthritis can cause activity-related aching and stiffness. A hot, red, suddenly painful knee may occur with Gout. A Baker's Cyst is usually felt behind the knee rather than directly over the cap, but knee fluid can make the whole area feel full.

Pause kneeling, deep squats, running, and other movements that increase discomfort. Elevate the leg when resting, and use a cold pack wrapped in cloth for short periods after a recent flare or injury. Do not press, puncture, or try to drain the lump yourself. Record whether the swelling is soft or tense, whether the knee bends fully, and whether the skin is warm or red. A clinician may examine the bursa and joint and consider imaging or Joint Aspiration when appropriate.
