What postpartum swelling can mean

Pregnancy increases body fluid, and intravenous fluids, reduced movement, and hormonal changes can add to puffiness after birth. Swelling in both feet or ankles may gradually ease with gentle movement and rest. It should not be assumed to be harmless if it is rapidly increasing or accompanied by other symptoms.

A clot can cause one-sided leg swelling, pain, warmth, or redness. A sudden headache, vision change, upper abdominal pain, breathlessness, or high blood pressure can signal a serious postpartum problem. Feeling persistently low, panicky, unable to sleep even when the baby sleeps, or unable to cope also deserves support; these are health concerns, not a personal failure.

What to do

Walk gently if your maternity team says it is safe, avoid standing still for long periods, and rest with your legs supported. Drink normally and do not take water tablets or supplements for swelling unless prescribed. Check any wound, breast, or catheter site for increasing redness, pain, or discharge.

Keep a note of when swelling began, whether it affects one or both sides, and any headache, vision, chest, or mood symptoms. Tell a trusted person if you are struggling emotionally so they can help you contact your maternity or primary-care team.

When to seek care

Seek emergency help for sudden breathlessness, chest pain, coughing blood, fainting, a seizure, severe headache with vision changes, confusion, or swelling of the face or tongue with breathing difficulty.

Contact your maternity team urgently for one-sided leg swelling or pain, rapidly worsening swelling, severe headache, visual disturbance, upper abdominal pain, fever, or very high blood pressure. Arrange a prompt mental-health assessment if sadness, fear, intrusive thoughts, or poor sleep is making daily care difficult; seek immediate help if you may harm yourself or the baby.
