Sunstroke symptoms and shivering

Shivering is involuntary muscle activity that produces heat. It may happen as the body responds to cooling, dehydration, or a rapid change in temperature. A person with heat stroke can be confused, weak, or very hot even while shivering, so the symptom should not be used to judge severity on its own.

Shivering can also accompany infections. Depending on the full pattern, clinicians may consider influenza, malaria, dengue fever, pneumonia, kidney infection, urinary tract infection, or sepsis. The symptom does not identify one cause by itself.

Move away from heat, remove excess clothing, and cool gradually with a fan, cool cloths, or a cool environment. Check alertness and breathing. Do not wrap someone in heavy blankets simply because they are shivering, and do not give drinks if confusion, vomiting, or reduced consciousness makes swallowing unsafe.

Record temperature if a reliable thermometer is available, note travel or insect exposure, and seek medical assessment rather than assuming the shivering is only from heat. Chills may be useful context for the clinician.

Call emergency services for confusion, collapse, seizure, severe breathing difficulty, blue or mottled skin, or extreme weakness. Get urgent medical care for persistent shivering with fever, severe headache, stiff neck, rash, chest pain, painful urination, flank pain, or recent travel to a malaria area. Heat exposure plus altered awareness is an emergency regardless of shivering.

Ask whether the pattern fits heat illness or infection, whether testing is needed, and how to manage fluids and temperature safely. Tell the clinician when shivering started, the measured temperature, recent travel, insect bites, urinary symptoms, cough, rash, and the cooling measures already tried.
