Sudden wheezing and shortness of breath: what to know

Wheezing is a high-pitched sound caused by air moving through narrowed airways. It may occur with asthma, chronic obstructive pulmonary disease, acute bronchitis, or bronchiectasis. Breathlessness can also arise when the heart, blood, or lungs cannot meet the body's oxygen needs; examples include angina, atrial fibrillation, and anaemia.

A sudden episode after eating, taking medicine, or an insect sting may be anaphylaxis, particularly with swelling of the lips or tongue, hives, hoarseness, or faintness. The sound and timing do not identify the cause by themselves. A clinician may ask about triggers, fever, cough, chest discomfort, known lung disease, and medicines, then check breathing and oxygen levels.

Stop exertion and sit upright in a position that makes breathing easier. Move away from smoke, dust, or a suspected allergen. If you have a prescribed reliever inhaler, use it according to your personal action plan; do not borrow someone else's inhaler.

Arrange same-day medical advice for a new episode, persistent wheeze, or breathlessness that returns. Keep track of when it started, what you were doing, and any cough, fever, rash, or chest symptoms. Avoid driving yourself if breathing is significantly difficult.

Call emergency services immediately if you are struggling to speak in full sentences, your lips look blue or grey, you become confused or faint, breathing is rapidly worsening, or there is severe chest pressure. Swelling of the mouth or throat after a possible allergen is also an emergency.

While waiting, stay with the person, keep them upright unless faint, and follow their prescribed emergency plan. Do not give food or drink to someone who is severely breathless or drowsy.
