What should you know about sudden sharp pain during sex?

Pain at the vaginal opening can follow dryness, skin irritation, pelvic-floor tightening, or conditions such as vulvodynia or lichen sclerosus. Deeper pain may involve the cervix, uterus, ovaries, or surrounding pelvic tissues. Endometriosis, an ovarian endometrioma, pelvic inflammatory disease, and genitourinary syndrome of menopause are possible explanations, but a symptom alone cannot identify the cause.

The timing gives useful clues: pain on entry differs from pain with deep penetration, and pain that starts suddenly can differ from pain that builds during sex. Bleeding, unusual discharge, fever, urinary symptoms, nausea, or pain outside sex should be mentioned during an assessment.

Stop the activity when pain begins and do not push through it. Rest, record the location and timing of the pain, and note any bleeding, discharge, fever, bowel or bladder changes, or relation to your menstrual cycle. Avoid inserting products or having sex again until the pain settles or you have advice.

Book a gynaecology or primary-care appointment when pain returns, affects intimacy, or has no clear short-lived trigger. An examination may check the skin, pelvic muscles, cervix, and abdomen; further tests depend on what is found. Treatment is directed at the cause, and may include addressing dryness, infection, inflammation, or pelvic-floor tension.

Seek urgent medical care for severe or persistent pelvic pain, faintness, heavy bleeding, fever, vomiting, a swollen or very tender abdomen, or pain with a possible pregnancy. Sudden severe pain with dizziness or shoulder-tip pain also needs prompt assessment. These warning signs can indicate a problem that cannot be evaluated safely at home.

Ask: Where does the pain seem to come from? Could dryness, pelvic-floor spasm, infection, or endometriosis explain it? Do I need an examination, pregnancy test, infection testing, or imaging? What activities should I pause, and what symptoms should prompt urgent help?
