What should you know about sudden increase in appetite and fatigue?

Short answer
A sudden increase in appetite with fatigue can reflect changes in sleep, stress, hydration, activity, medicines, or an underlying health problem, so persistent or worsening symptoms deserve medical assessment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What this combination may mean
Appetite and energy can shift together when sleep is disrupted, meals are irregular, physical activity changes, or emotional strain increases. Dehydration may leave you tired and can be confused with hunger, especially in hot weather. Alcohol use can disturb sleep and affect appetite the following day; Alcohol Use Disorder can present with changes in sleep, appetite, and energy.
Fatigue is a common presentation of anaemia, coeliac disease, COVID-19, and Chronic Fatigue Syndrome, but these conditions have different patterns and accompanying symptoms. Chronic kidney disease may also cause tiredness. A change in appetite by itself cannot identify the cause, and fatigue does not mean that any one of these conditions is present.
What to do now
For several days, note when hunger and fatigue occur, your sleep quality, fluid intake, meals, activity, mood, recent illness, and any new medicine or supplement. Eat regular meals with protein and fibre, drink fluids regularly, and avoid using energy drinks to push through marked tiredness. Do not stop a prescribed medicine without advice.
Arrange a routine appointment if the change continues, interferes with work or daily activities, or comes with weight change, thirst, frequent urination, bowel changes, breathlessness, paleness, fever, or swollen ankles. A clinician can review the timeline, examine you, and decide whether tests are appropriate.
When to seek urgent care
Seek urgent medical help for severe or rapidly worsening weakness, confusion, fainting, trouble breathing, chest pain, inability to keep fluids down, or signs of severe dehydration such as very little urine and dizziness that does not settle. Get prompt advice for a new severe headache, one-sided weakness, or unusual drowsiness.
If fatigue follows a recent infection, recovery may take time, but worsening symptoms, a high fever, or difficulty breathing should not be attributed to recovery alone. Dehydration and COVID-19 are possible explanations in the right context, while symptoms after eating gluten may warrant discussion of Coeliac Disease.
Questions for your doctor
Ask: What changes could explain my appetite and energy pattern? Could my medicines, sleep, hydration, mood, or recent illness be contributing? Which symptoms should I monitor? Do my symptoms suggest checking for anaemia, coeliac disease, kidney problems, or another cause? What should I do if the fatigue becomes harder to manage?

Questions people ask
Does increased appetite explain fatigue?
Not necessarily. Hunger and fatigue can occur together after poor sleep, stress, dehydration, illness, or changes in routine, and the combination does not identify one diagnosis.
Should I see a doctor about sudden hunger and tiredness?
Arrange medical advice if the change persists, worsens, affects daily life, or comes with weight change, thirst, frequent urination, breathlessness, fever, or bowel symptoms.
What information should I bring to an appointment?
Bring the start date, a symptom diary, recent illness details, changes in sleep or activity, and a list of medicines and supplements.
