Sudden headache during sex: what to know

Some headaches build as sexual excitement and exertion increase. Others begin abruptly at orgasm. Muscle tension, dehydration, heat, poor sleep, or holding your breath can contribute, but the same pattern can also occur with a problem affecting blood vessels in or around the brain. The timing and intensity matter more than the fact that sex triggered it.

A headache that reaches its maximum intensity within moments, feels unlike previous headaches, or follows neck stiffness, vomiting, faintness, confusion, weakness, trouble speaking, or vision change should not be self-diagnosed. A routine viral illness or sinus problem may cause head pain, but it does not explain away a thunderclap pattern.

Stop sexual activity, sit or lie somewhere safe, and note when the pain began, how quickly it peaked, where it is located, and any associated symptoms. Do not drive yourself if you feel faint, confused, weak, or unusually unwell. Avoid restarting activity until a clinician has assessed a first sudden severe episode.

If the pain is mild, settles fully, and resembles a previously assessed exertional headache, arrange a medical review to discuss triggers and prevention. Call emergency services or attend an emergency department immediately for a first or worst-ever sudden headache, pain that peaks instantly, loss of consciousness, seizure, fever with a stiff neck, repeated vomiting, new weakness or numbness, difficulty speaking, severe dizziness, or a change in vision. Seek urgent help after a head or neck injury or if symptoms continue or worsen.

Ask whether the onset requires emergency imaging, whether blood-pressure or neurological checks are needed, and which symptoms should trigger emergency help.
