What should you know about subcutaneous calcaneal bursa pain?

Short answer
Subcutaneous calcaneal bursa pain is tenderness in the superficial heel cushion, commonly worsened by shoe pressure, friction, or direct contact.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Location
- The bursa sits between heel skin and the calcaneus.
- Pressure source
- Firm shoe backs and repeated rubbing can aggravate symptoms.
- Warning sign
- Heat, spreading redness, fever, or drainage needs prompt care.
What it means
The calcaneus is the heel bone. A subcutaneous bursa lies between the skin and the back of that bone, where footwear and repeated rubbing can irritate it. Pain may be local, tender to touch, and associated with a small soft swelling. Achilles tendon problems, skin injury, infection, gout, or another bursa can cause similar symptoms, so the exact position matters. Joint swelling may be described as heel swelling even when the joint is not the source.
Ways to reduce irritation
Choose footwear that does not press on the back of the heel, reduce rubbing, and pause activities that repeatedly load or strike the area. A clinician can check the skin, bursa, Achilles tendon, and ankle movement. Anti-inflammatory medication may be considered when safe for you. A cortisone injection or joint aspiration is not automatically suitable for a superficial heel swelling; the diagnosis and infection risk should be reviewed first. Avoid trying to puncture or massage a hot, very tender lump.
When to seek care
Seek prompt medical attention if the heel becomes increasingly red or hot, swelling spreads, fever develops, fluid drains, or pain becomes severe. Arrange assessment after a wound, bite, or significant injury, or when walking becomes difficult. Sudden inability to push off the foot needs urgent evaluation.
Questions for your doctor
Is the pain coming from the superficial bursa, Achilles tendon, skin, or joint? Could infection be present? What footwear or activity changes are useful? Would medication, injection, or aspiration be safe at this site?

Questions people ask
Is superficial heel bursitis the same as Achilles tendinitis?
No. They are different structures, although their pain can overlap around the back of the heel.
Should I wear open-backed shoes?
They may reduce direct pressure for some people, but choose stable, comfortable footwear and stop if walking worsens pain.
