Can subcision treat ice-pick scars?

Short answer

Subcision is designed for tethered depressed scars and may not be the right treatment for narrow ice-pick scars.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

Why scar shape matters

Ice-pick scars are small openings that extend deeply into the skin. Subcision uses a needle or similar instrument beneath the surface to release bands that tether a depressed scar; it does not simply fill a narrow, deep channel. Rolling scars are more often associated with this tethering pattern, whereas ice-pick scars may need a technique directed at the individual opening or surface texture. A face can have more than one scar type, so a clinician may map the scars rather than use one treatment everywhere. Acne Scar Laser Treatment may be discussed when uneven surface texture is also present.

What to do before treatment

Arrange an assessment with a qualified dermatology clinician who can examine the scar edges, depth, skin tone, and any active acne or inflammation. Bring a list of prior procedures and products, and mention medicines or supplements that affect bleeding or healing. Avoid picking at the scars. If subcision is selected for a tethered component, follow the aftercare instructions for cleansing, sun protection, and temporary activity limits. Read the Subcision and Acne Scar Laser Treatment pages to understand how those options differ.

When to seek care

Ask the clinic to review severe or increasing pain, spreading redness, marked warmth, pus-like drainage, fever, or swelling that continues to intensify. Urgent care is appropriate for breathing difficulty or rapidly progressive facial swelling. A new dark or pale patch, persistent numbness, or a wound that is not settling also deserves prompt review.

Questions for your doctor

Ask whether each scar is ice-pick, rolling, boxcar, or mixed; what treatment is aimed at the depth of the scar; whether subcision would address any tethering; and how the clinician will judge progress. Ask about expected downtime, pigment changes, infection, and options if the first approach does not suit the scar structure.

Questions people ask

Does subcision remove an ice-pick scar?

Subcision may not reach the main structure of an ice-pick scar, so another scar-directed method may be considered.

Can different scar treatments be combined?

A clinician may combine or sequence treatments when a person has different scar types, with healing time between procedures.
