What should you know about subcision and TCA CROSS?

Subcision is performed beneath the skin to separate fibrous bands holding a rolling or bound-down depression. TCA CROSS means applying a high-strength trichloroacetic acid solution in a targeted way within ice-pick scars. The acid causes a controlled injury that can encourage remodelling inside the pit; it is not a general facial peel. A scar may have more than one pattern, so a clinician may select different techniques for different areas.

Swelling, redness, crusting, tenderness, bruising, and temporary pigment changes are possible. Acid placed beyond the intended pit can irritate surrounding skin and increase the chance of colour change or scarring, so this is not a home procedure.

During consultation, ask the clinician to identify rolling, boxcar, and ice-pick components separately. Mention recent sun exposure, active breakouts, eczema, medicines, and any tendency to form raised scars. Follow the planned cleansing and sun-protection routine. Let crusts shed naturally, avoid picking, and do not add exfoliating acids or unapproved lightening products while the skin is healing. Review is useful before repeating or combining procedures.

Call the treating clinic if pain, redness, warmth, drainage, or swelling is increasing, or if fever develops. Seek prompt medical assessment for spreading chemical irritation, extensive blistering, marked skin colour change, or signs of infection. Do not neutralise or apply another acid unless the treating clinician gives specific instructions.
