What should you know about subchondral stress fracture?

Short answer
A subchondral stress fracture is a small crack or collapse risk in bone beneath joint cartilage that needs protected loading.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Location
- The injury lies beneath joint cartilage.
- Typical clue
- Pain often increases with loading and may cause a limp.
- Protection
- Follow the prescribed weight-bearing plan until reassessed.
What it means
Subchondral bone sits immediately below the smooth cartilage at a joint. Repeated impact, a sudden increase in activity, altered walking mechanics, or reduced bone strength can overload it. Pain is often focused near the affected joint and may worsen with standing or walking. Depending on the site, people may describe foot pain, heel pain, shin pain, or thigh pain. Limping is a useful sign that the painful area needs assessment rather than continued loading.
An X-ray can be normal early in the process. A clinician decides whether further imaging is needed from the examination, symptoms, and risk factors. Vitamin D deficiency is one possible contributor to reduced bone health, but it is not the only explanation.
What to do now
Stop the activity that triggers the pain and arrange an orthopaedic assessment. Until reviewed, use the walking aid or protected weight-bearing plan you are given; do not test the injury by repeatedly walking through pain. Supportive footwear and avoiding impact may help, but they do not replace diagnosis. Recovery plans usually progress loading gradually after pain and examination findings improve. If testing identifies vitamin D deficiency, vitamin D supplementation should follow an individual medical plan rather than self-prescribed high doses.
When to seek care
Seek urgent assessment if you cannot bear weight, pain becomes severe at rest, swelling or bruising increases quickly, the limb looks deformed, or the foot or leg becomes cold, pale, blue, or numb. A fall or direct blow followed by sudden pain also warrants prompt evaluation.
Questions for your doctor
Where is the fracture and how much weight can I place on it? Do I need imaging beyond an X-ray? Should bone-health tests include vitamin D? What symptoms mean I can begin gradual activity?

Questions people ask
Can a subchondral stress fracture heal?
Many improve with timely diagnosis, protected loading, and a gradual return plan, but the site and severity affect management.
Should I keep exercising through the pain?
No. Stop impact activity and obtain advice about safe movement and weight bearing.
