Stupp schema for glioblastoma: what to know

How the structures involved in Glioblastoma differ from normal

Short answer

The Stupp schema for glioblastoma combines tumour surgery when possible with radiotherapy and chemotherapy, followed by ongoing treatment planning.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Core components
Surgery, radiotherapy and chemotherapy may be combined.
Personalisation
The plan changes with pathology, health and recovery.

What the schema describes

Glioblastoma is an aggressive brain tumour whose treatment usually involves more than one discipline. The schema refers to a sequence in which surgery may obtain tissue and remove as much visible tumour as safely possible, followed by Radiotherapy given with a chemotherapy medicine, then further chemotherapy in selected patients. The exact schedule, dose and suitability depend on age, general health, neurological function, molecular findings, residual tumour and recovery after surgery. The schema is a framework, not a promise that every person will receive identical care.

Preparing for treatment

Ask the team to map the proposed sequence onto your own scan and pathology results. Brain Tumour Surgery may be aimed at diagnosis, symptom relief or safe removal. Discuss steroid use, seizure medicines, rehabilitation, fatigue, nausea, blood monitoring and how treatment may affect daily activities. Keep a record of new symptoms and medicines, and ask who to contact outside appointments. Clinical Trials may be relevant at diagnosis or recurrence, depending on eligibility and local availability.

When to seek urgent help

During or after treatment, urgent assessment is needed for a new seizure, sudden weakness or speech difficulty, severe headache with vomiting, confusion, reduced alertness, fever, or a wound that becomes increasingly red or drains. Do not wait for the next scheduled visit when neurological symptoms appear suddenly.

Questions for your doctor

What does my pathology and molecular report show? What is the aim of surgery? How will Radiotherapy and Chemotherapy be sequenced? Which side effects need same-day advice? Are any Clinical Trials appropriate for me?

Questions people ask

Does every person with glioblastoma follow the Stupp schema?

No. The approach may be unsuitable or modified according to health, neurological function, tumour features, recovery and personal goals.

What is the role of surgery in this approach?

Surgery can provide tissue for diagnosis and may remove visible tumour when this can be done safely; the intended role varies between patients.
