Strong headache in pregnancy: what to know

Pregnancy can bring headaches through sleep disruption, dehydration, missed meals, stress, congestion, or a viral illness. However, a severe or changing headache can also occur with a pregnancy-related blood-pressure problem, so its timing and accompanying symptoms matter. Headache pain alone cannot identify the cause.

Write down when the pain began, whether it was sudden, how long it lasts, where it is located, and whether you notice flashing lights, blurred vision, pain under the ribs, unusual swelling, vomiting, weakness, or confusion. Mention any history of migraine or high blood pressure.

Rest somewhere quiet, drink fluids, eat regular small meals, and use a cool cloth if it helps. Before taking any painkiller, confirm the choice and dose with your maternity clinician, doctor, or pharmacist; medicines that are suitable outside pregnancy may not be suitable during it. Do not start a combination cold remedy without checking its ingredients.

Contact your maternity team or clinician the same day if the headache persists, returns repeatedly, is not relieved by the plan they gave you, or feels unlike your usual headaches. Take any home blood-pressure reading with you, including the time it was taken.

Seek emergency care for sudden severe pain, seizure, fainting, confusion, weakness, difficulty speaking, new loss of vision, severe breathlessness, or a headache after an injury. Get urgent maternity assessment for severe headache with flashing lights or blurred vision, marked swelling of the face or hands, severe upper abdominal pain, repeated vomiting, or feeling seriously unwell.

Ask which pain-relief options are appropriate for your stage of pregnancy and health, whether your blood pressure needs checking, and which symptoms require immediate attendance. Share all prescription medicines, supplements, and over-the-counter products so the plan can be checked safely.
