Stress lump on back of neck: what to know

Stress can tighten the muscles across the neck and shoulders, creating a tender knot or a feeling of fullness. A true lump may instead be a swollen lymph node, a skin cyst, a fatty lump, or another neck mass. A neck lump is a common presentation of several conditions, and its presence alone does not identify the cause. Assessment may include looking at the skin, feeling the area, and checking the mouth, throat, thyroid and nearby lymph nodes. Depending on the findings, a clinician may consider imaging or a sample.

Notice when the area appeared, whether it moves under the skin, and whether it changes with neck movement or relaxation. Avoid repeatedly squeezing it. Gentle movement, heat and a review of posture may ease muscle tightness, but they should not replace an examination if the lump remains. Keep a note of fever, night sweats, weight change, voice change, swallowing difficulty or recent infection. A clinician may investigate conditions such as Head and Neck Cancer, Lymphoma, Thyroid Cancer or Cancer of Unknown Primary when the examination calls for it.

Arrange a medical review for a lump that persists, grows, feels hard or fixed, returns, or comes with unexplained symptoms. Seek urgent help for trouble breathing, rapidly increasing swelling, inability to swallow saliva, or marked weakness. A new neck lump should not be attributed to stress solely because the neck feels tense.
