What should you know about stress induced heart failure?

Short answer
Stress does not by itself establish heart failure, but it can worsen blood pressure, heart rhythm and symptoms in someone with heart disease.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Possible symptoms
- Breathlessness, fatigue, cough, palpitations or swollen ankles that are new or worsening.
- Medication rule
- Do not alter heart medicines because of stress without clinical advice.
Stress and heart function
Heart failure means the heart is not pumping or filling well enough for the body’s needs. Long-term hypertension, atrial fibrillation and other heart conditions can contribute. Emotional or physical strain may increase the heart rate and blood pressure, disturb sleep and make breathlessness or tiredness feel worse; it is not a diagnosis by itself. Possible heart-failure symptoms include breathlessness, fatigue, cough, palpitations and swollen ankles, especially when they are new or worsening.
What to do next
If you already have heart failure, continue prescribed medicines and follow your clinician’s fluid, salt and activity advice; do not change doses because of stress without guidance. Note changes in breathing, energy, cough, heartbeat, weight trend or ankle swelling, and arrange a review when symptoms change. A chronic disease review can bring medication checks, blood pressure assessment and a discussion of practical support.
When to seek urgent care
Call emergency services for severe or rapidly worsening breathlessness, chest pressure, blue or grey lips, collapse, new confusion, or palpitations with faintness. Seek same-day care for breathlessness at rest, rapidly increasing ankle swelling, a new persistent cough, or a clear decline in exercise ability. These symptoms need assessment even when stress is present.
Questions for your doctor
Ask whether your symptoms could reflect fluid build-up, a rhythm problem or another cause, and which changes require emergency help. Ask how to monitor symptoms at home and when your next chronic disease review should occur.

Questions people ask
Can stress cause heart failure?
Stress alone does not diagnose heart failure, but sustained strain can worsen blood pressure, rhythm problems and symptoms in people with heart disease.
Are swollen ankles always heart failure?
No. Swelling has several possible causes; its timing, extent and accompanying breathlessness need clinical assessment.
