Stress fracture of the third metatarsal

How the structures involved in Stress Fracture differ from normal

Short answer

A third metatarsal stress fracture commonly causes focused midfoot pain; treatment protects the foot from impact while the bone heals and loading returns gradually.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Typical location
Focused pain may sit over the middle of the forefoot.
Protection
A stiff shoe, boot, or crutches may be advised.
Progression
Impact returns only after comfortable everyday loading and clinical guidance.

What the third metatarsal does

The third metatarsal is one of the long bones between the midfoot and the toes. Repeated force from running, jumping, long walks, marching, or a rapid change in footwear can create a stress injury. Pain is often concentrated over the middle of the forefoot and may be accompanied by swelling or limping. It can start during activity and later interfere with ordinary steps.

A bruise, joint irritation, tendon problem, or another metatarsal injury can feel similar. An examination may identify the tender bone, while imaging is chosen according to the suspected injury and how long symptoms have been present. Foot Pain that keeps returning after rest should not be dismissed as normal training soreness.

How it is usually managed

Stop impact activity and reduce walking until you have advice about the injury. A clinician may recommend a stiff-soled shoe, walking boot, or crutches, depending on tenderness and weight-bearing tolerance. Elevate the foot when swollen and use a wrapped cold pack briefly if it helps. Avoid tight footwear, forceful massage over the painful spot, and repeated hopping to check whether it is better.

After symptoms settle and healing is assessed, rehabilitation can address calf and foot strength, balance, and walking mechanics. Return to running or jumping should be gradual, changing one element at a time. If pain returns at the metatarsal, return to the last comfortable level and contact your clinician. Vitamin D Supplementation is considered when a deficiency is identified, not as a substitute for rest.

When to seek care

Arrange prompt care for focal tenderness over the metatarsal, swelling that persists, pain during normal walking, or a limp. Seek urgent help if the foot is deformed, numb, cold, discoloured, rapidly more swollen, or unable to bear weight. Fever with redness and warmth needs urgent review because infection can require different treatment. Recurrent metatarsal stress injuries also warrant a broader review of training and bone health.

Questions for your doctor

Ask whether the third metatarsal is the likely injured bone, whether imaging is needed, and how much walking is safe. Clarify the right shoe or boot, when to begin foot exercises, and which milestones allow a return to impact. Ask whether repeated injuries call for Vitamin D testing, nutrition advice, or a review of training progression.

Questions people ask

Can a third metatarsal stress fracture cause swelling?

Yes, local swelling can occur, particularly after loading. Swelling is not specific, so persistent focal pain needs assessment.

Can I wear normal shoes with this injury?

Some people need a stiff-soled shoe or boot rather than a flexible normal shoe. Follow the support and weight-bearing plan given after assessment.
