What it means

Sesamoids are small bones embedded in the tendons under the front of the foot. They help the big toe bend and absorb pressure during walking, running and jumping. Repeated impact can irritate the bone before a crack develops. Pain is usually focused under the big-toe joint and may increase when the toe bends upward or when you push off. Swelling, tenderness and a changed gait can occur. A normal early X-ray does not always exclude a stress injury, so the clinician may combine an examination with follow-up imaging.

What you can do

Stop the activity that triggers pain and use a stiff-soled shoe, boot or other support only as advised. Do not test the area by repeatedly jumping or running. Ice wrapped in cloth can help settle discomfort, and elevation may reduce swelling. Your clinician may recommend protected weight-bearing while the bone heals, followed by gradual mobility and strength work. Return to sport should progress from comfortable walking to low-impact exercise and then higher impact, with symptoms guiding each stage. Discuss nutrition and whether testing for Vitamin D Deficiency is appropriate; supplementation should be individualized.

When to seek care

Arrange an assessment for persistent pain beneath the big toe, pain that returns whenever you load the foot, or limping. Seek urgent help if the foot becomes cold, pale, numb or markedly swollen, if you cannot bear weight, or if there is a new deformity. Prompt review also matters when pain is worsening despite rest, because continued loading can enlarge the injury.
