What should you know about strep without sore throat?

Some people with strep have fever, swollen neck glands, headache, nausea, a rash or inflamed tonsils without describing a strong sore throat. Children may show irritability, reduced appetite or difficulty swallowing instead. Symptoms vary with age and with the site of infection, so the absence of throat pain does not identify the cause.

Note the timing of fever, exposure to someone with a diagnosed infection, swallowing difficulty, rash, cough and nasal symptoms. Drink enough to keep your urine pale, rest, and use suitable symptom relief according to the label or professional advice. Do not start leftover antibiotics. A clinician can examine the throat and decide whether a test or treatment is appropriate.

Seek medical advice if fever or swollen glands continue, symptoms are worsening, or the person is struggling to drink. Get urgent help for breathing difficulty, drooling, inability to swallow, severe drowsiness, confusion, a rapidly spreading rash or signs of dehydration.

Ask what findings make strep more or less likely, whether testing is useful despite little throat pain, what other causes fit the symptoms, and which changes should prompt urgent review.
