What should you know about stomach cramps and no appetite?
Stomach cramps and no appetite can result from digestive irritation, infection, stress, medicines, or another health condition; the pattern and accompanying symptoms help guide next steps. Cramps may feel like squeezing, twisting, or intermittent pain in the upper or lower abdomen. A short-lived change in appetite can happen when nausea, fever, diarrhoea, constipation, or pain makes eating unpleasant.
What these symptoms can mean
Digestive infections such as gastroenteritis may cause cramps with nausea, vomiting, diarrhoea, or fever. Stomach-lining irritation, including gastritis, and a peptic ulcer can cause upper-abdominal discomfort, nausea, and reduced interest in food. Viral illnesses such as influenza or COVID-19 may also lower appetite while causing symptoms outside the digestive system. Hepatitis A can include appetite loss, nausea, abdominal discomfort, dark urine, or yellowing of the skin and eyes. Emotional strain and depression can change appetite and digestion. Persistent or worsening symptoms need assessment because less common conditions, including pancreatic cancer, can also present with appetite loss and abdominal symptoms. These symptoms alone cannot identify a cause.
What you can do now
Take small, frequent sips of water or an oral rehydration drink, especially if you have vomiting or diarrhoea. Try small portions of bland food when you can tolerate them, and avoid alcohol, very fatty meals, and foods that clearly worsen the cramps. Rest and record when the pain occurs, what you eat, bowel changes, fever, vomiting, medicines, and any recent travel or sick contacts. Do not stop a prescribed medicine without advice, but ask a pharmacist or clinician whether a medicine could be contributing. Arrange a medical review if the appetite change continues, pain keeps returning, or you are eating and drinking much less than usual.
When to seek medical care
Seek urgent medical help for severe or rapidly worsening abdominal pain, a swollen or rigid abdomen, fainting, confusion, trouble breathing, blood in vomit or stool, black tar-like stool, or repeated vomiting that prevents fluids staying down. Get prompt assessment for yellow skin or eyes, very dark urine, signs of dehydration such as very little urine or marked dizziness, or pain with a high fever. Contact a clinician soon if symptoms persist, appetite loss is unexplained, you are losing weight without trying, or you have a history of digestive disease, abdominal surgery, or immune problems. Children, older adults, and pregnant people may need earlier advice when they are unable to eat or drink normally.
Questions for your doctor
You could ask: What causes fit the location and pattern of my cramps? Could any medicine, supplement, infection, or emotional change be affecting my appetite? Which symptoms should prompt urgent care? Do I need an examination or tests, and what should I eat and drink while recovering? Bring a list of medicines and a brief symptom diary so the clinician can assess the timing, severity, and associated changes.
