What loss of appetite means during a stomach bug

Gastroenteritis commonly presents with digestive symptoms such as nausea, vomiting, loose stools, stomach cramps and a reduced desire to eat. Fever, tiredness or body aches can also make food less appealing. Appetite often returns gradually as vomiting and diarrhoea settle. A temporary drop in food intake is usually less urgent than being unable to replace lost fluids.

Loss of appetite is not specific to a stomach bug. Similar symptoms can occur with other infections or digestive conditions, including influenza, COVID-19, gastritis, peptic ulcer disease or hepatitis A. Persistent appetite loss without typical stomach-bug symptoms needs a separate assessment, particularly if it comes with ongoing pain, yellowing of the skin or eyes, or unintended weight loss.

What you can do

Take frequent small sips of water or an oral rehydration drink. If vomiting makes drinking difficult, try a sip every few minutes and increase slowly when it stays down. Do not force a full meal. When nausea eases, try small portions of foods such as rice, toast, bananas, potatoes, soup or yoghurt if tolerated. Gradually return to your usual diet as hunger improves.

Rest, wash your hands carefully and avoid preparing food for other people while you may be infectious. Avoid alcohol and foods that clearly worsen nausea or diarrhoea. Children, older adults, pregnant people and anyone with a long-term medical condition can become dehydrated more quickly, so use a lower threshold for contacting a clinician.

When to seek medical care

Seek urgent medical help if you cannot keep fluids down, feel faint or confused, pass very little urine, have severe or worsening abdominal pain, vomit blood, or notice blood or black material in your stool. A dry mouth, marked weakness and dizziness when standing can also signal dehydration.

Arrange medical advice if appetite does not begin to return after the stomach symptoms improve, vomiting or diarrhoea continues, fever is persistent, or you have a condition or medicine that increases dehydration risk. A clinician can examine you and decide whether testing or treatment for another cause is needed.
