Still pregnant after miscarriage: what should you know?

Pregnancy hormone does not disappear immediately when a pregnancy ends, so a home test may stay positive for a while. A positive result can also mean the pregnancy has continued, the miscarriage is incomplete, or a new pregnancy has begun if ovulation returned. Bleeding and cramping patterns can help, but they cannot confirm the cause. A Transvaginal Ultrasound can show whether a pregnancy is developing in the uterus and whether tissue remains. Your clinician may also use repeat hormone tests and your symptoms to interpret the result.

Contact the clinician or early-pregnancy service that managed the miscarriage and tell them when the bleeding started, whether it is improving, and when the test was taken. Do not rely on repeated home tests alone. Keep a record of pain, bleeding, fever, dizziness, and any new pregnancy symptoms. Follow the recommended scan or blood-test plan. If tissue remains, options may include waiting with monitoring, Medical Management of Miscarriage, or Surgical Management of Miscarriage; the suitable choice depends on your symptoms and scan findings.

Seek urgent care for severe or one-sided abdominal pain, shoulder-tip pain, faintness, heavy bleeding, fever, or feeling suddenly unwell. These symptoms can signal significant bleeding, infection, or a pregnancy outside the uterus. Prompt assessment is also needed if pain or bleeding is worsening rather than settling, or if you cannot keep fluids down.

Ask whether the test result is expected for your stage of recovery, which follow-up test is needed, and when it should be repeated. Ask what the scan showed, whether tissue remains, and which warning symptoms mean you should return immediately.
