Steroid for chest infection: what to know

Chest infections can irritate the breathing tubes and cause cough, mucus, fever or wheezing. Steroids reduce inflammation; they do not kill bacteria or viruses. A clinician may consider them when wheezing is prominent, asthma or chronic lung disease is flaring, or the airway swelling is making breathing difficult. A simple viral illness such as a common cold or uncomplicated acute bronchitis may not benefit from a steroid. The correct treatment depends on an examination and the pattern of symptoms.

Do not use leftover tablets, share an inhaler, or seek an injection without assessment. Arrange a GP consultation if the cough is troublesome, breathing feels tight, wheezing is new, or symptoms are not settling. Follow the prescribed dose and duration exactly if a steroid is recommended. Rest, drink fluids, avoid smoke, and use simple comfort measures advised by your clinician. Ask whether a nebuliser treatment is appropriate if inhaled medicine is difficult to use.

Seek urgent medical help for severe breathlessness, blue or grey lips, chest pain, confusion, fainting, or difficulty speaking because of breathing. Prompt assessment is also sensible for a persistent high fever, coughing up blood, dehydration, or worsening symptoms after initial improvement. A cough with phlegm can occur in several illnesses, so its presence alone does not show that a steroid is needed.

Ask what is causing the chest symptoms, whether airway narrowing is present, and what benefit a steroid is expected to provide. Check whether an inhaled option is preferable, how to use it, which side effects to watch for, and when you should arrange a review.
