Sternocleidomastoid Muscle Strain: What to Know

Short answer
A sternocleidomastoid muscle strain can cause one-sided neck pain, tenderness, stiffness, and discomfort when turning your head.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Main area
- Side of the neck, sometimes toward the shoulder or upper chest
- Typical trigger
- A sudden or sustained neck movement
What this strain means
The sternocleidomastoid is a paired muscle running from the breastbone and collarbone to the base of the skull behind the ear. It helps bend, rotate, and steady the neck. Sudden lifting, awkward sleep, prolonged screen posture, or a forceful head movement can overload its fibres. Pain may spread toward the shoulder, upper chest, jaw, or behind the ear, and the muscle may feel tight when you gently look away from the sore side.
A strain is different from ordinary muscle soreness after exercise, although the symptoms can overlap. A clinician considers the movement that triggered the pain, neck motion, tenderness, strength, and nerve symptoms. See [Muscle Strain](/conditions/muscle-strain/) for broader information.
What you can do
For the first day or two, reduce activities that reproduce the pain while keeping the neck moving gently within a comfortable range. A wrapped cold pack may calm soreness; later, gentle warmth can help some people relax tight muscles. Support your neck in a neutral position during sleep, bring screens closer to eye level, and avoid forceful stretching, deep massage, or heavy lifting until movement is easier. A physiotherapist can guide gradual mobility and strengthening when symptoms settle.
When to seek care
Arrange medical assessment if pain followed a collision, fall, or other significant force; if it is severe, worsening, or not settling; or if you cannot turn your head normally. Seek urgent help for neck pain with arm or leg weakness, numbness, trouble walking, severe headache, fever, confusion, difficulty swallowing, or breathing difficulty. These features can point beyond a simple muscle strain.
Questions for your doctor
Ask what examination findings support a muscle strain, which movements are safe now, whether physiotherapy would help, and what changes should prompt review. Mention any injury, new medicines, fever, or symptoms traveling into an arm.
