What should you know about statin dark urine?

Urine can become darker when you are dehydrated, after heavy exercise, or because of food or another medicine. However, dark or cola-coloured urine with muscle symptoms can signal muscle injury, and dark urine with yellow skin or eyes can point to a liver or bile problem. The timing, shade, and accompanying symptoms help a clinician judge the cause.

Statins lower cholesterol, but rarely they can be linked with significant muscle irritation or breakdown. Muscle pigment released into the blood can pass into urine and make it look dark. This concern is greater when dark urine occurs with new muscle pain, tenderness, weakness, fever, or reduced urine.

Dark urine can also accompany dehydration or heat exhaustion, particularly after sweating, vomiting, diarrhoea, or time in hot conditions. Liver inflammation, including hepatitis, is another possible explanation and may cause pale stools, itching, nausea, or yellowing of the skin or whites of the eyes. These symptoms do not identify the cause on their own.

Contact the prescriber or an urgent medical service today for advice, and describe when the urine changed, how dark it is, your statin name and dose, recent exercise, fluid intake, and every other medicine or supplement you use. Do not restart, stop, or change the statin without specific advice unless an urgent clinician tells you to do so.

Drink fluids if you can do so safely, avoid strenuous exercise until assessed, and keep a note of urine colour and related symptoms. A clinician may check your muscles, hydration, liver, kidney function, and blood tests to identify the safest next step.

Seek urgent care now for dark urine with severe or spreading muscle pain, marked weakness, confusion, fainting, very little urine, or inability to keep fluids down. Call emergency services for breathing difficulty, collapse, or severe swelling of the face or throat.

Obtain prompt medical assessment for yellow skin or eyes, severe abdominal pain, persistent vomiting, fever with worsening illness, or urine that remains dark despite drinking normally. Take your medicine packaging or an up-to-date list of medicines with you.

Ask whether your symptoms could be related to the statin, dehydration, heat exhaustion, liver inflammation, or another medicine; whether you need blood and urine tests; and when it is safe to take the next dose. Ask what muscle or urine changes should trigger immediate help in the future.
