What should you know about stage 4 lung cancer shortness of breath?

Stage 4 lung cancer means the cancer has spread beyond its original area. Breathlessness may relate to the tumour, fluid around the lung, an airway that is narrowed, infection, anaemia, a blood clot, or a heart or lung condition. Lung cancer presents with shortness of breath in some people, but breathlessness is not specific to lung cancer and does not identify its cause on its own.

Describe when it happens: during movement, while lying down, during sleep or at rest. Also mention cough, chest pain, fever, wheezing, blue lips, ankle swelling and any change from your usual breathing. A clinician may check oxygen levels, listen to your chest and arrange tests to find a treatable cause.

Contact your oncology or respiratory team promptly about new or worsening breathlessness. Sit upright, loosen tight clothing and use any prescribed inhaler, oxygen or other treatment exactly as directed. Keep a note of triggers, timing and associated symptoms. Do not begin someone else’s inhaler or oxygen. Your team may review cancer treatment, drain fluid, treat infection, address anaemia or provide breathing and symptom support, depending on the cause.

Call local emergency services for severe breathlessness, breathing difficulty that starts suddenly, inability to speak in full sentences, blue or grey lips, fainting, new confusion, severe chest pain, or coughing up blood. Do not drive yourself if you are struggling to breathe. Sudden breathlessness can have an urgent cause even when cancer is already known.

Ask: What is the most likely cause of my breathlessness? Do I need an urgent scan or blood test? Could fluid, infection, anaemia or a clot be contributing? Which medicines or breathing supports should I use? What exact changes mean I should call emergency services?
