What should you know about stage 4 liver metastases?

Liver metastases are different from a cancer that started in the liver. The primary cancer may be bowel cancer, pancreatic cancer, breast cancer, or another condition. Scans and tissue testing help identify the origin and show how many areas are involved.

Treatment usually focuses on the cancer of origin and may use chemotherapy or another medicine that travels through the bloodstream. If disease is limited to suitable areas, liver resection or tumour ablation may be discussed. The liver’s reserve, the position of deposits, and cancer elsewhere all affect whether a local procedure is possible.

Ask your team to state the primary cancer, the treatment goal, and how response will be checked. Keep a list of medicines and report changes in appetite, weight, bowel habits, abdominal discomfort, itching, urine colour, or energy. Tell clinicians about liver disease, blood thinners, supplements, and previous cancer treatments.

Supportive care can be provided with cancer treatment. It may help with pain, nausea, nutrition, sleep, fatigue, anxiety, and planning at home. A referral does not mean that active cancer treatment has ended.

Contact the cancer team promptly for increasing jaundice, fever, repeated vomiting, a swollen or very painful abdomen, unusual bleeding, or new confusion. Seek emergency help for vomiting blood, black stools, fainting, severe abdominal pain, or marked drowsiness that is difficult to wake from.

Where did the cancer start, and where else is it present? What is the aim of treatment? Is chemotherapy appropriate for the original cancer? Could liver resection or tumour ablation be considered? How will liver function and treatment response be monitored? Which symptoms require a same-day call?
