Stabbing pain during sex: what to know

Sharp pain at the vaginal opening may relate to vulvodynia, lichen sclerosus, irritation, or dryness associated with genitourinary syndrome of menopause. Deeper pain can occur with endometriosis, an ovarian endometrioma, or pelvic inflammatory disease. Pain during sex is a symptom, not a diagnosis, and its timing, depth, cycle pattern, discharge, and urinary symptoms help guide the evaluation.

Stop or change activity when pain begins; do not push through a stabbing sensation. Note whether pain is at entry or deep inside, and whether it occurs with bleeding, discharge, fever, or a particular position. A clinician may examine the vulva and vagina, assess the pelvis, and arrange tests or imaging based on your history. Treatment depends on the cause and may include targeted medicine, pelvic-floor care, or specialist treatment.

Seek urgent help for severe sudden pelvic pain, faintness, fever, vomiting, heavy bleeding, or pain with a possible pregnancy. Arrange a prompt appointment if the pain keeps returning, is worsening, causes bleeding, or comes with unusual discharge or pain when passing urine.

Ask whether the pain is superficial or deep, which causes fit its pattern, whether examination or imaging is needed, and what intimacy options can reduce discomfort while you are being assessed.
