Why it can happen

The cervix can bleed more easily during pregnancy, including after sex or an examination. Other causes include an infection, a small bleed near the pregnancy, or a pregnancy complication. Spotting may settle, but its appearance cannot confirm that a pregnancy is progressing normally. Cervical conditions can also cause bleeding, so the pregnancy and cervix may both need consideration. Pain on one side, shoulder-tip pain, or faintness is particularly concerning because it can signal internal bleeding.

What to do after noticing blood

Contact your maternity team, obstetric clinician, or an urgent care service for individual advice, even if the spotting is light. Record the colour, amount, clots, timing, pain, and whether it followed sex. Keep your pregnancy details and blood group information available if known. Assessment may involve an examination, urine or blood testing, and ultrasound depending on gestational timing and symptoms. Use pads rather than tampons while bleeding so the amount is easier to observe.

When to seek urgent help

Go to urgent care immediately for heavy or rapidly increasing bleeding, severe or one-sided abdominal pain, shoulder-tip pain, fainting, collapse, marked dizziness, or feeling very unwell. Fever or foul-smelling discharge also needs prompt assessment. Do not drive yourself if you feel faint. A small amount of spotting without pain still warrants advice from the pregnancy-care team.
