Spotting During Post Menopause: What Should You Know?

After periods have stopped, even a small pink, brown, or red mark on underwear or toilet paper counts as bleeding and deserves medical advice. Lower oestrogen can make vaginal tissue thinner and more easily irritated, especially after sex; vaginal dryness may therefore contribute. Polyps, inflammation, some medicines, and pelvic organ prolapse can also be involved. Bleeding can sometimes come from the cervix, bladder, or rectum rather than the vagina, so identifying the source matters. A womb condition is another possibility, which is why the symptom should not be explained away as part of menopause.

Note the date, colour, amount, whether it followed sex, and symptoms such as pain, pressure, discharge, itching, or urinary changes. Hot flushes, night sweats, mood changes, low sex drive, or vaginal dryness may help describe the menopausal context but do not explain away bleeding.

Contact a GP or gynaecology service even if the spotting happens once and stops. Take a list of medicines and hormone treatments, and mention when your periods stopped, previous cervical screening, and any pelvic symptoms. The clinician may examine the vagina and cervix and may recommend tests or an ultrasound to look at the womb lining. Do not insert products or have sex if these worsen bleeding or discomfort until you have advice.

Seek urgent help if bleeding is heavy or persistent, or if it comes with severe pelvic pain, faintness, weakness, fever, or shortness of breath. Urgent assessment is also appropriate if you cannot tell whether the blood is vaginal or comes from urine or the bowel and you feel unwell. Keep the appointment even if the bleeding settles.
