Spotting During Early Pregnancy: What to Know

Spotting means light vaginal bleeding, such as pink, red, or brown marks on toilet paper or underwear. It can occur after sex or an internal examination because the cervix may bleed more easily during pregnancy. It can also occur when a pregnancy is not developing normally, or with a pregnancy outside the womb. The amount, colour, timing, and symptoms alongside the bleeding help a clinician decide what evaluation is needed.

Light spotting does not by itself show whether a pregnancy is healthy. Some people have spotting and continue with a healthy pregnancy, while bleeding can also be an early sign of miscarriage or ectopic pregnancy. Ectopic pregnancy occurs when the pregnancy grows outside the womb and can become an emergency if internal bleeding develops.

Other explanations can include irritation of the cervix, infection, or bleeding from tissue in the vagina. A condition such as Chlamydia may cause bleeding after sex or unusual discharge. Use a pad rather than a tampon so you can observe the amount, and avoid putting anything into the vagina until you have received advice if bleeding is continuing.

Assessment may include questions about your pregnancy, an examination, blood tests, and an ultrasound. The timing of an ultrasound matters because an early scan may not yet show all expected findings. Do not stop prescribed medicines or start treatment based on spotting alone; ask the clinician caring for your pregnancy what is appropriate for you.

Seek emergency care now for heavy or rapidly increasing bleeding, severe or one-sided lower abdominal pain, shoulder-tip pain, fainting, marked dizziness, weakness, or trouble breathing. These symptoms can signal significant blood loss or an ectopic pregnancy. Fever, worsening pain, or feeling acutely unwell also needs prompt assessment because infection may require treatment.
