Common and less common explanations
The timing may fall near ovulation, when a temporary hormone change can cause a small amount of discharge. Starting, stopping, or missing hormonal contraception can also disturb the lining. Irritation after sex, vaginal dryness, or a cervical infection may cause blood to appear outside the expected period. Chlamydia can present with bleeding between periods, sometimes alongside discharge, pelvic pain, or pain during sex.

A pregnancy test may be appropriate when pregnancy is possible.
When to get urgent help
Seek urgent care for heavy or rapidly increasing bleeding, severe abdominal or pelvic pain, one-sided pain, faintness, shoulder pain, fever, or unusual weakness. Bleeding with possible pregnancy and significant pain needs prompt assessment for ectopic pregnancy. Foul-smelling discharge with pain or fever also warrants urgent review because infection may need treatment.
