Common and less common explanations

The timing may fall near ovulation, when a temporary hormone change can cause a small amount of discharge. Starting, stopping, or missing hormonal contraception can also disturb the lining. Irritation after sex, vaginal dryness, or a cervical infection may cause blood to appear outside the expected period. Chlamydia can present with bleeding between periods, sometimes alongside discharge, pelvic pain, or pain during sex.

Cervical dysplasia, cervical cancer, endometrial polyps, uterine fibroids, and womb cancer can present with unexpected bleeding. Spotting itself does not tell you which condition, if any, is present. A clinician can use your cycle history and examination to decide whether testing is needed.

How to decide your next step

Write down the date your period ended, the day spotting began, its colour and amount, and any relation to sex, contraception, or missed pills. Track pain, discharge, odour, and fever as well. A one-off small episode may settle, but book a routine appointment if the bleeding repeats, lasts, follows sex, or is a new change in your usual cycle. A pregnancy test may be appropriate when pregnancy is possible.

When to get urgent help

Seek urgent care for heavy or rapidly increasing bleeding, severe abdominal or pelvic pain, one-sided pain, faintness, shoulder pain, fever, or unusual weakness. Bleeding with possible pregnancy and significant pain needs prompt assessment for ectopic pregnancy. Foul-smelling discharge with pain or fever also warrants urgent review because infection may need treatment.
