Spotting after miscarriage: what to know

Light pink, red, or brown discharge may follow the passage of pregnancy tissue. The flow should generally trend down rather than become heavier. Cramping can accompany the uterus contracting. Bleeding that continues, returns after settling, or comes with unpleasant-smelling discharge can indicate that tissue remains, infection is developing, or another cause needs checking. A pregnancy test may remain positive for a while, so symptoms and examination matter more than one home result.

A clinician may use a pelvic assessment, blood testing, or a transvaginal ultrasound to check the uterus and confirm whether the miscarriage has completed. The scan uses an internal probe to view the uterus and surrounding structures.

Note the colour and amount of bleeding, the presence of clots, your pain, temperature, and any dizziness. Use pads while bleeding so changes are easier to see, and avoid putting anything into the vagina until a clinician advises that it is appropriate. Rest, drink fluids, and use only pain relief that is suitable for you. Arrange follow-up if spotting is not easing or if you are unsure whether the miscarriage has fully passed.

If tissue remains, treatment may involve waiting with follow-up, medicines that help the uterus pass it, or a procedure. Your clinician can explain medical management of miscarriage and surgical management of miscarriage according to your symptoms and preferences.

Seek urgent medical help for heavy or rapidly increasing bleeding, severe or one-sided abdominal pain, shoulder pain, faintness, marked weakness, fever, chills, or discharge with a strong unpleasant smell. These signs can accompany significant blood loss, infection, or an ectopic pregnancy and should not be judged by spotting alone. If you know you are Rh-negative, ask promptly whether you need anti-D treatment.

Ask whether the bleeding pattern is expected, whether you need an ultrasound or blood test, how to recognise incomplete miscarriage or infection, when to resume sex and exercise, and when it is medically reasonable to try for another pregnancy.
