Spotting 24 hours after sex: what to know

Spotting 24 hours after sex can follow cervical irritation, but recurring bleeding or other symptoms needs medical assessment. Friction, dryness, or a small surface injury may cause light staining, yet bleeding after sex can also occur with infection, cervical cell changes, or a growth. Note what happened and arrange advice with a GP, gynaecologist, or sexual health clinic if it continues, returns, or concerns you.

The cervix and vaginal tissues can bleed after contact, particularly when lubrication is limited or the tissue is irritated. A vaginal or cervical infection may cause discharge, odour, burning, pelvic pain, or pain during sex; Chlamydia can sometimes cause bleeding after sex. Cervical dysplasia and cervical cancer can also present with bleeding after sex, although spotting by itself cannot identify the cause. Polyps or fibroids may contribute to bleeding from the reproductive tract, and pregnancy-related bleeding has its own assessment needs.

A clinician may ask about contraception, pregnancy possibility, periods, new partners, pain, discharge, and whether the bleeding happened before. Testing may include a pregnancy test, infection testing, a speculum examination, or cervical screening depending on your history and screening status.

Use a pad or liner to see whether the spotting stops, and record its colour, amount, timing, and any associated pain or discharge. Pause penetrative sex until bleeding has settled or you have received advice, especially if sex is painful. Avoid inserting tampons while bleeding. If pregnancy is possible, take a pregnancy test at an appropriate time and mention the result when seeking care. Do not douche, because it can irritate tissue and make symptoms harder to assess.

Seek urgent care for heavy or increasing bleeding, severe lower abdominal or pelvic pain, faintness, marked dizziness, fever, or feeling acutely unwell. Get prompt medical advice if you have a positive pregnancy test with bleeding or pain, or if bleeding follows a missed period. Arrange a non-urgent appointment when spotting happens repeatedly, occurs after sex more than once, or comes with unusual discharge, pain, or bleeding between periods.

Ask whether the bleeding pattern suggests irritation, infection, or a cervical problem; which tests are appropriate; and whether your cervical screening is up to date. Tell the clinician about pregnancy possibility, contraception, medicines that affect bleeding, pain, discharge, and any new sexual partner. Ask when it is safe to resume sex and which symptoms should bring you back sooner.
