What should you know about a sore lump on the back of your neck?

Tenderness can occur when nearby tissue is inflamed or a lymph node is reacting to an infection. A blocked hair follicle, boil, cyst, muscle irritation, or recent skin injury can also create a painful swelling. The location, size, firmness, movement, skin changes, and how long it has been present all help a clinician assess it.

A neck lump can be a presentation of head and neck cancer, lymphoma, thyroid cancer, or cancer of unknown primary, but a sore lump does not establish any of these diagnoses. A clinician may examine your mouth, throat, skin, thyroid area, and lymph-node regions, then decide whether imaging or a sample is appropriate.

Note when you first noticed the lump and whether it changes with illness, swallowing, head movement, or touch. Check for fever, sore throat, dental symptoms, ear symptoms, skin redness, drainage, voice change, or unexplained weight change. Avoid squeezing or repeatedly pressing it, because that can irritate the area.

Arrange a medical assessment if the lump remains, enlarges, feels firm or fixed, or returns after seeming to settle. Bring a list of medicines and recent infections or dental problems. If the assessment suggests a thyroid source, the clinician can explain options such as thyroid removal surgery when relevant.

Seek urgent help for trouble breathing or swallowing, rapidly increasing swelling, severe weakness, or a high fever with spreading redness. Arrange prompt review for a lump that persists, grows, is hard or immobile, or comes with persistent hoarseness, night sweats, or unexplained weight loss. These features call for assessment; they do not by themselves identify cancer.

Ask: Where does the lump seem to arise? Does its tenderness suggest inflammation? Do I need an examination of my throat, mouth, skin, or thyroid? Would imaging or a tissue sample clarify it? What changes should make me return sooner?
