Small red spots on skin: what to know

Location and texture provide useful clues. Spots around hairs may fit folliculitis, while clogged pores can form pimples. A rough, itchy patch may behave more like dermatitis. Consider recent shaving, sweating, a new cosmetic, medicine, insect exposure, or sun exposure. Check whether the spots blanch when gently pressed, form a ring, scale, crust, blister, or bruise, and whether they are spreading. These observations help a clinician narrow the possibilities without assuming that one appearance has one cause.

Clean the area with a mild, fragrance-free product and avoid picking or scrubbing. Keep sweaty skin dry, change out of damp clothing, and pause shaving over inflamed areas. Introduce only one new skincare product at a time, so a reaction is easier to recognise. Do not use steroid creams on an undiagnosed rash unless advised. Take a clear photo in good light if the spots change quickly; it can help document the pattern.

Get urgent medical help when spots are accompanied by trouble breathing, facial swelling, faintness, severe illness, or a rapidly spreading rash. Prompt assessment is also needed for purple or bruise-like spots that do not fade with pressure, fever, significant pain, pus, blistering, or involvement of the eyes or mouth. A non-urgent appointment is sensible when spots persist, recur, scar, or resist gentle care.
