What should you know about a small lump on the back of the neck?

The back of the neck contains skin, hair follicles, muscles, and chains of lymph nodes. A node may swell in response to irritation or infection of the scalp, skin, throat, or nearby areas. A blocked skin gland, cyst, fatty tissue, or muscle-related change can also feel like a small lump. Lymphoma and head and neck cancer can present with neck nodes in some people, but the position of a lump does not diagnose either condition. Thyroid lumps are usually toward the front, although symptoms can be felt more broadly around the neck.

Ask someone to help you identify the exact spot if it is difficult to see. Note whether the surface is red, whether the lump is tender or warm, and whether it slides under the skin or seems attached.

Arrange an appointment if the lump remains, grows, returns, or has no clear link to a recent skin or respiratory problem. A clinician may inspect the scalp and skin, examine the throat and ears, and feel lymph nodes throughout the neck. Ultrasound can help separate a superficial skin lump from a deeper node; a blood test or fine-needle sample may be recommended when the findings call for it. Do not squeeze, lance, or repeatedly rub the swelling, as this may inflame the area.

Seek urgent care if swelling causes breathing or swallowing difficulty, expands quickly, or comes with severe pain and feeling very unwell. Seek prompt medical advice for a hard or fixed lump, steady enlargement, a lump above the collarbone, persistent hoarseness, unexplained weight loss, night sweats, or ongoing fever. A small size does not rule out the need for assessment when these features are present.

Ask whether the lump feels like a lymph node, skin cyst, or another structure, and whether the scalp, ears, throat, or teeth could be contributing. Ask if ultrasound is appropriate, whether you should measure or photograph it, and how quickly to return if it changes.
