Skin rash from a necklace: what to know

Metal, plating, clasp components, perfume, sunscreen, sweat, and repeated rubbing can irritate the neck or trigger Contact Dermatitis. A rash that follows the chain or pendant and appears after wearing it supports that possibility. Redness, itch, scaling, or small bumps can also occur with Atopic Dermatitis, Heat Rash, Intertrigo, Psoriasis, Ringworm, Scabies, or Drug Rash. The timing of the rash and whether it improves away from the necklace can help a clinician distinguish them.

Remove the necklace and gently rinse the neck, especially after sweating or using fragrance. Keep the area dry, avoid rubbing and scratching, and wear a soft collar or loose neckline while it settles. Clean the necklace separately and do not test it again on irritated skin. List the necklace material, any new cosmetics, and the timing of symptoms for your appointment. A pharmacist or clinician can recommend a suitable product if itching is troublesome.

Seek medical advice if the rash returns with exposure, spreads beyond the contact area, or persists after the necklace is removed. Prompt review is needed for increasing pain, warmth, swelling, pus, crusting, or fever. Get emergency help for breathing difficulty, facial or tongue swelling, faintness, or widespread hives.
