What should you know about skin graft take?

A graft has no immediate blood supply of its own. During healing, the wound bed supports the graft first, followed by the growth of small blood vessels into it. The graft must remain closely opposed to the wound surface so fluid, movement, or a collection of blood does not separate it.

Early changes can include swelling, altered colour, mild tenderness, and oozing under a dressing. Appearance alone cannot confirm whether the graft is taking; the surgical team may inspect it at planned dressing changes.

Keep the dressing in place and dry for the period your surgical team prescribed. Avoid rubbing, stretching, pressure, or picking at crusts. Limit activity that pulls across the graft, and follow instructions for elevation, medicines, bathing, and care of the donor site.

Do not apply creams, antiseptics, or home remedies beneath the dressing unless the treating team has told you to do so. Healing needs vary with the graft location, its thickness, and the wound beneath it.

Contact the surgical team promptly if the dressing becomes soaked, slips, or exposes the graft, or if pain, swelling, drainage, redness, or odour is increasing. Seek urgent help for heavy bleeding, fever with worsening wound symptoms, sudden separation, or difficulty breathing.

Ask when the first dressing change is planned, which colour changes are expected, how much movement is safe, and how the donor site should be covered. Ask who to call outside clinic hours if the dressing loosens or the graft is injured.
