The right approach depends on whether the concern is colour contrast, a raised or tight surface, uneven texture, itching, tenderness, or a scar that limits movement. Both the grafted area and the donor site can leave scars, so an assessment should consider each area separately.

A skin graft is skin moved from one part of the body to cover another area. The graft may heal with a boundary that differs in colour, thickness, texture, or hair growth from surrounding skin. Its appearance can also change as healing and scar remodelling continue.

A scar may become firm when collagen builds densely, or feel tight where healing skin crosses a joint or other moving area. Colour changes may be more noticeable after sun exposure. Some symptoms, such as itch or sensitivity, arise from the healing scar itself and deserve attention even when the appearance is not the main concern.

Fractional laser treatment may be considered for selected scars; surgery is generally reserved for a scar that needs release, repositioning, or another structural change. Seek prompt medical advice if the graft or donor-site scar becomes increasingly red, warm, swollen, painful, or starts draining fluid. A newly opening area, bleeding that does not settle, fever, or a rapid change in the scar also needs assessment.

Arrange a review if tightness is making it hard to move a nearby joint, or if persistent itch, pain, or sensitivity is disrupting sleep or daily activities. These concerns can guide treatment even if the scar looks settled from a distance.
